Rates
$150.00 for the initial session; $140.00 per session thereafter
I would be happy to help you sort out insurance issues, including dealing directly with your insurance company regarding your coverage and benefits.
Insurance
I accept a number of insurances (eg. Messa and Messa PPO (in-network provider), BCBS and BCBS PPO (in-network), Blue Care Network (in-network) Priority Health (in-network), Set/Seg, Aetna (in-network), Cofinity (in-network), PPOM, Physician's Care, Benefit Source, Meritain, Weyco, NGS, Meijer Real Health, HHS, Sisco and several others. Some of these insurances cover my services in-network and some on an out of network basis.
Services may be covered in full or in part by your health insurance or employee benefit plan. Please check your coverage carefully by asking the following questions:
- Do I have mental health benefits?
- What is my deductible and has it been met?
- How many sessions per calendar year does my plan cover?
- How much does my plan cover for an out-of-network provider?
- What is the coverage amount per therapy session?
- Is approval required from my primary care physician?
Reduced Fee
Reduced fee services are available on a limited basis.
Payment
Cash and checks are accepted for payment, as well as Visa, Mastercard and Discover.
Cancellation Policy
If you do not show up for your scheduled appointment, and you have not notified ume at least 24 hours in advance, you will be required to pay a 50% late cancellation/no show fee.
